As a hypnotherapist, I am affiliated with the Nederlandse Beroepsvereniging van Hypnotherapeuten (NBVH) (Dutch Professional Association of Hypnotherapists), with registration number 11028.
registrations as a healthcare provider
The practice is registered under the name Praktijk voor Zelfonderzoek with the national information center for health insurers Vektis, and is included in the General Data Management for Healthcare Providers (AGB).
registration as a business with the Chamber of Commerce
The practice is registered with the Chamber of Commerce (Kamer van Koophandel) under the name Praktijk voor zelfonderzoek, with KvK number: 530152310000
complaints procedures for clients
Should you be dissatisfied with something and we are unable to resolve it together, you can contact my professional association (hypnotherapie.nl).
There, you can call on a complaints officer as prescribed by the Dutch Healthcare Quality, Complaints and Disputes Act (Wkkgz).
In accordance with this law, I am also affiliated with the disputes body of the SCAG.
I am registered in the Register of Practitioners of Complementary Care (RBCZ), with license number 403040R. This umbrella organization is responsible for disciplinary law.
